The DGT Form 04 is mandatory when obtaining a driving permit or license, and the requirements for obtaining a driving permit or license are as follows:
- Residence in Spain (students must prove that they have been living in Spain for at least six months).
- Not to be banned by court order from driving motor vehicles and mopeds, nor to be subject to suspension or administrative intervention.
- Meet the psychological and physical aptitudes required in relation to the type of license applied for.
- Be of the required age for each permit or license (Vid. Art. 4 of the General Regulations for Drivers).
To learn the conditions of the issuance for each type of license, it will be necessary to comply with the provisions of Article 7 of the General Regulations for Drivers.
The attached form is to be submitted by the driving school in cases where a license is being applied for the first time and is also used for the following procedures:
- Obtaining and/or extending the authorization to transport dangerous goods.
- License recovery due to loss of points.
