How can I Become a Self-Employed Doctor in Spain?
Here we bring you the step-by-step process of everything you have to do to become a self-employed doctor (or "médico autónomo") in Spain if you are a non-EU citizen.
Homologation of your degree (recognized by the Ministerio de Educación) and homologation of your specialization (by the Ministerio de Sanidad). Then register in the College of Physicians of the province you live in.
Apply for a work permit whether from your country of residence or Spain (under a tourist visa). These can be a visa for self-employment or as a highly-qualified worker.
Obtain your Foreigner Identity Number (NIE) after you arrive in Spain and apply for the Foreigner Identity Card (TIE) by means of your work permit.

Register as an autónomo, by registering in the Special Regime for Self-Employed Workers (RETA) of the Social Security, or the MEL(Médico Ejercicio Libre) by Mutual Médica. Simultaneously, register with the Tax Agency.
Additional Info to Keep in Mind
- As you will work on your own as a médico autónomo you do not need to retake a residency exam nor take the MIR to start practicing. You will only need to have your credentials accepted by Spain.
- You must be fluent in Spanish in order to practice medicine once your medical license has been recognized.
- During the first year since your registration as an autónomo, you can apply for a flat fee with Social Security, paying only €80 per month.
- As a self-employed doctor, you are not required to file taxes on a quarterly basis. However, you must comply with the submission of a withholding invoice (between 7 or 15%) to the Tax Agency in your annual Income Tax Return.
