(National Identity Document -DNI-, Foreign Identity Card or Passport).<\/li>\n\n\n\n<li><strong>The number of the Identity Document.<\/strong><\/li>\n\n\n\n<li><strong>Social Security number<\/strong>: In the case of a data change request, the worker&#8217;s Social Security number (also called &#8220;affiliation number&#8221;) must be specified in the Form.<\/li>\n\n\n\n<li><strong>Degree of disability<\/strong>: if you are a person with a disability, the degree of disability should be indicated in accordance with the assessment report made by the IMSERSO or by the competent body of the Autonomous Community.<\/li>\n\n\n\n<li><strong>Address<\/strong>: you must indicate the type of road (street, square, path, passage, etc.) and the name of the public road, without abbreviations.<\/li>\n\n\n\n<li><strong>Municipality and province<\/strong>.<\/li>\n\n\n\n<li><strong>Telematic Data<\/strong> (telephone and email): The recording of this data implies the acceptance of informative notifications from Social Security.<\/li>\n<\/ul>\n\n\n\n<p>&nbsp;<\/p>\n\n\n\n<p><strong>Information related to the application<\/strong> (you must mark an X in the corresponding box):<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Affiliation to Social Security<\/strong>.<\/li>\n\n\n\n<li><strong>Assignment of the Social Security number<\/strong>.<\/li>\n\n\n\n<li><strong>Data modification<\/strong>.<\/li>\n<\/ul>\n\n\n\n<p>&nbsp;<\/p>\n\n\n\n<p><strong>-Data related to the notifications<\/strong>:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Place, date and <strong>signature of the applicant<\/strong>.<\/li>\n\n\n\n<li>Place, date and <strong>signature of the employer<\/strong> (if applicable).<\/li>\n<\/ul>\n\n\n\n<p>&nbsp;<\/p>\n\n\n\n<p>Subsequently, you will be given a receipt for completing the Social Security Affiliation Application, Assignment of a Social Security Number, and Data Modification (Form TA1).&nbsp;The data displayed in this form will be incorporated into the <strong>General Affiliation File<\/strong> (&#8220;Fichero General de Afiliaci\u00f3n&#8221;).<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Who can request Form TA1?<\/strong><\/h2>\n\n\n\n<p>The <strong>Social Security Affiliation Number<\/strong>, as well as registration to the system, may be requested by the following entities\/people:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>The company that is going to <strong>hire <\/strong>a worker <strong>who does not have a Social Security Number<\/strong>, since it is obliged to do so.&nbsp;If the company does not carry out the procedure, the employee may also request it.<\/li>\n\n\n\n<li><strong>Self-employed workers<\/strong> who start an economic activity and are not registered yet.<\/li>\n\n\n\n<li>The <strong>Provincial Directorates of the General Treasury of Social Security<\/strong> or its Administrations, when they have proof that a worker is not registered.&nbsp;For example, in the event that a labor inspector detects it.<\/li>\n<\/ul>\n\n\n\n<p><\/p>\n\n\n\n<p>The Application Form TA1 <strong>must be addressed to the Provincial Directorate of the General Treasury of Social Security<\/strong> or to the Administration of the province where the company in which you work is registered.&nbsp;Likewise, you can submit it to the tax address of the&nbsp;<a href=\"http:\/\/entretramites.com\/en\/services\/advice-self-employed\" target=\"_blank\" rel=\"noreferrer noopener\"><strong>self-employed<\/strong><\/a> worker.<\/p>\n\n\n\n<p>Keep in mind that the request must be made before the employee begins to provide their services to a company.&nbsp;It can be done 60 calendar days prior to the work year.<\/p>\n\n\n\n<p>We hope this information has been useful to you!
